Gan Ami Emergency Contact and Pick Up Authorization – Must be completed one week prior to start date, then updated every new contracted school year.

Family Intake Questionnaire  – Must be completed one week prior to start date

Intake for Children Under Two Years – Must be completed one week prior to start for a child under the age of Two (2) years old and Updated every three months until the child turns Two Years Old

Health History – Must be completed one week prior to start date and updated only if information changes

Child Care Enrollment Form – Must be completed one week prior to start date and updated only if information changes

Child Health Report – Must be Returned by your child’s start date. Doctor’s signature is required.
Children Under Two (2) years need an updated form and signature every six (6) months.
Children Over Two (2) years need an updated form and signature every two (2) years.

Child Care Immunization Record – Must be returned by your child’s start date. Parent or Guardian signature is required on this form.
If a printed immunization record is used for immunization documentation, the Immunization Record Report MUST still accompany the documentation with Section One and Five completed and signed.
Children Under Two (2) years need an updated form and signature every six (6) months.
Children Over Two (2) years need an updated form and signature every two (2) years.

Medication Authorization Form – Only required if child needs to receive medication at school. Be detailed and specific. Form can ONLY be dated for a maximum of two (2) weeks in length at a time. ALL Medication and Creams MUST come to school in original packaging.

Upon completion, forms can be emailed to Early Childhood Business Manager
